Daily Self-Attestation

In our ongoing effort to protect our school and community, we ask that all staff and students complete this self- assessment each day before coming to school. 
 
Please review the symptoms below. If you are experiencing any of these symptoms that cannot be explained by allergies or a non-infectious cause (such as a chronic condition), you must stay home. 
 
  • Cough not due to known causes 
  • Muscle or body aches
  • Sore throat
  • Fever (100 Fahrenheit or higher)
  • Difficulty breathing; shortness of breath
  • Headache in combination with other symptoms
  • Nasal congestion/runny nose (not due to other known causes) w/other symptoms.  
  • Chills
  • Fatigue in combination with other symptoms
  • New loss of taste or smell
  • Nausea/vomiting/diarrhea
 
By entering the school, you are attesting the following statements to be true and accurate: 
 
  • I do not have any symptoms of Covid-19. 
  • I have not had close contact (less than six feet for more than 15 minutes) with a person with Covid-19. 
  • I am adhering to the most recent MA travel guidelines in response to Covid-19. The advisory will be enforced.
  • I will follow the GLCPS Covid-19 protocols. 
  • I will wear a face mask while in the building in accordance with GLCPS Face Mask Protocol. 
 
If you cannot attest to the above statements, you must stay home.
 
  • For staff, please follow the typical call out procedures and await further guidance.
  • For students, please contact the school nurses
Visiting GLCPS? Before you enter the building, our front office staff will ask you two questions: 
 
"Do you have any Covid-19 symptoms?"
 
"Have you been exposed to someone with Covid-19 in the past ten days?"
 
If you answer YES to either question, you cannot enter the building. 
 
If you answer NO, you will be allowed into the building and asked to complete a self-attestation form located inside the main entrance. You will be asked to bring the completed form to the front desk and place it in the box labeled "Completed Attestation Forms." 
 
If you are planning to visit the building and would like to complete the form before you arrive, you can do so by downloading and printing it.
